The Performance Dashboard will display data for the performance measures attributed to your pharmacy within a specific measurement period. The adjudicated pharmacy claims data that EQUIPP® receives can be found in the performance dashboard relevant to the specific medication-related quality measures.

Utilizing Analyze Performance to Impact Your Goals

Within Analyze Performance, your pharmacy’s quality champion can review how your scores match up to the goals of the health plans aligned with your patients.

Improving on Patient Outliers

Patient outliers within EQUIPP® are those not meeting the intent of the quality measure they’re included in. Understanding the patient’s barrier(s) and engaging with them to get back on track with their medication therapies will be the key component of improving your performance scores and your patient’s health outcomes.

Evaluating Your Progress in “My Programs”

For some health plans partnering with EQUIPP®, the data for their payer programs your pharmacy is participating in can be found in the “My Programs” tab. The specific measures, patient outliers, and in some events, financial incentive opportunities can be found in this section.

Enhancing Engagement with Pharmacy Staff and Patients

Pharmacies have such a strong connection to their patients and work closely with their providers to improve their medicinal regimen. In using EQUIPP®, your pharmacy staff and quality champion can understand which patients need improvement and create a plan on how they can be positively impacted.

SETTING YOUR TEAM UP FOR SUCCESS

Each member of the pharmacy team can make a positive influence on improving performance on quality measures. The impact will also lend improvement in payer programs that may be tied back to potential bonus dollars. EQUIPP® updates its performance information mid-month, every month, and has some outlier updates as often as weekly. Logging in once a month or as often as once a week for as little as 20 minutes in the platform at a time can make an impact in achieving your pharmacy’s goals.
