AI Blog Summary

Your pharmacy team already counsels patients, follows up on refills, and closes gaps in care. EQUIPP connects that work to bonus-only, payer-funded programs, so the care you deliver every day earns revenue on top of dispensing. PQS walked through how it works at the fall 2026 Total Pharmacy® Solutions Summit, and the full session is now available on demand. Here are the highlights.

Bonus-Only Incentives

Every EQUIPP program is bonus-only. Pharmacies are paid for documented performance on top of normal dispensing revenue, with no downside risk and no clawback for a missed opportunity.

‍

Bonuses are layered by activity and quarter:

‍

  • Quarterly check-ins (up to 3 per patient per measure: Q2, Q3, Q4) toward an 80%+ PDC target for diabetes, hypertension (RASA), and cholesterol.
  • Care gap closures for SUPD and Asthma Controller (once per patient per year).
  • On-time fill bonuses on 30-day and extended-day supplies, once a check-in is logged.

‍

The All Patients view puts an estimated dollar value on every open opportunity, so you know which patients to reach first. That $90 profile broke down to $10 for a Q3 check-in, $20 for an EDS (extended days' supply) conversion, and $60 for a final fill. Three opportunities, one patient. Now picture that across every patient with an open opportunity on your list. In the on-demand session, Rosalyn shows exactly where to find those dollars in the All Patients view and how to work through them before the year closes.

The Neutral Layer Between Plans and Pharmacies

PQS works with national and regional health plans and PBMs, reaching 65,000+ pharmacies (95% of U.S. community pharmacy locations). EQUIPP turns claims data into care-gap opportunities your pharmacy can act on: PQS pulls medical and Rx claims from client health plans and PBMs, calculates measures from stewards like PQA and NCQA down to the NPI level, and surfaces the gaps tied to a plan's incentive structure,.

EQUIPP Program Categories

Programs map back to PQA and NCQA measures, so you know exactly what's rewarded.

‍

  • Medication adherence. Diabetes, hypertension (RASA), and cholesterol PDC through quarterly and monthly check-ins, final fill, and 90-day conversion.
  • Preventive gap closure. Controlling Blood Pressure (CBP) and HbA1c readings captured at the point of dispensing. PQS manages contracting and incentive payments to pharmacies and supports HEDIS® submissions directly.
  • Formulary adherence. Switching eligible patients identified by health plans inside EQUIPP to preferred, lower-copay medications, with progress tracked in EQUIPP My Programs.  Pharmacies earn incentives when patients continue filling the preferred formulary alternative after the switch.

Opioid and benzodiazepine safety, polypharmacy (Poly-ACH), and immunization programs are coming soon.

Adherence Workflow
Knowing which programs pay is only half the picture. Bonuses are earned through the work your team does each week, and they’re paid only when that work is documented. Here’s how it moves from a patient list to a payment:

  1. Identify. Click the bullseye icon to open the Enhanced Services dashboard, where CBP, HbA1c, and LAMP patient lists refresh every Friday alongside what you've earned year to date, what's still available to earn, and earnings you've missed.
  2. Engage. Pharmacists and Pharmacy Technicians work the list in EQUIPP or the EQUIPP Copilot flyout during normal workflow.
  3. Document. Interventions are logged in the EQUIPP dashboard to satisfy payment and validation requirements.
  4. Get paid. Payment flows through the pharmacy's PSAO once PQS receives funds from the health plan.

Building a Plan

Documented care shows up in the numbers. Patients with a documented intervention were on average 16 percentage points more adherent than patients without one, and pharmacies using EQUIPP Copilot closed 1.5 to 2 times as many year-end adherence gaps as documentation-only users. For your pharmacy, higher adherence is what moves patients toward the 80% PDC target that bonuses reward.

Results like these come from structure, and the session laid out a four-part plan to build it into your week:

  1. Start with the highest-value work. Prioritize quarterly check-ins, CBP and HbA1c capture, and formulary adherence.
  2. Build a weekly rhythm. Give the Friday list an owner at the counter, so it gets worked every week.
  3. Track your progress. Watch PDC and gap-closure rates against the 80% year-end goal.
  4. Convert care into revenue. Document every intervention immediately. Undocumented care doesn’t convert into bonus payments, no matter how good the outcome..

With Q4 check-ins and final fill opportunities open now, it's a practical guide to the year-end bonus revenue still available. Watch it on demand from the fall 2026 Total Pharmacy® Solutions Summit.
