
If your pharmacy team only has time for one routine in EQUIPP, this is the one worth protecting: check-ins, on-time fill, and final fill. Each step builds on the last, and together they help you close adherence gaps for your patients and capture revenue you've already earned.
Quarterly check-ins are the foundation of the routine, and the cycle breaks down into four steps:
The routine centers on three core PDC measures: diabetes, hypertension, and cholesterol, each with a target of 80% or higher by year end. Patients can have up to three check-ins (1 per quarter) per measure. If you're not sure where a patient stands, the list is built to tell you.
While working the list, keep these things in mind:
Here's the weekly version of this workflow: log in and see pending check-in opportunities in the EQUIPP dashboard, identify patients with outstanding check-ins while watching expiration dates, complete each check-in before it expires, and fill the linked prescription before the intervention date passes. That last step is what makes the payment eligible.
A completed check-in is what makes the refill that follows eligible for on-time fill revenue, whether it's a 28-day supply or an 84-plus-day supply, as long as the quarterly check-in was completed before the refill due date. There's no extra documentation required. Mark the check-in complete in EQUIPP, and the rest follows.
The final fill of the year for a patient who has reached their adherence goal carries its own bonus, again with no added work on your part. Plans pay a bonus for that final refill once the interaction is documented. The advice here is the same as with check-ins: sort by expiration date and work the opportunities in order of those which expire first.
This is timely. Final fill opportunities for Q3 and Q4 may already be appearing. You'll find them under LAMP, inside Enhanced Services, following the same familiar pattern: identify, engage, document, and fill. Sort by expiration date, document your interaction with the patient, refill the medication, and make sure it leaves the pharmacy before the expiration date.
A new patient intervention status label now surfaces directly on a patient's opportunity, before your team takes action. Its main purpose is to help prevent outreach to patients who have passed away. When a team member marks a patient as deceased while working an opportunity, that status stays visible as a tag, so if another opportunity populates for that patient later, your team has the context before reaching out again while the plan works through removing the patient from the platform. It's available across both EQUIPP Copilot and EQUIPP web, with no extra steps required.
Check your notifications bell, the orange icon in the top right of your dashboard. It shows a count of unread messages, and those notifications are a weekly reminder of new opportunities and the dollars tied to them.
Log in to EQUIPP and clear every outstanding Q3 check-in before September 30. Enhanced Services refreshes every Friday, so the sooner your team works through the list, the fewer opportunities expire as Q4 begins. Once check-ins are handled, that same habit carries straight into on-time fill and final fill. A little consistency each week is what makes the whole program work.
