  • Quarterly Check-ins: Regular communication to support adherence.
  • Extended Day Supply (EDS) Conversions: Transitioning eligible patients from 30-day to extended day supplies.
  • SUPD and SPC Conversions: Further opportunities to optimize medication dispensing by closing statin-related gaps in care.
  • Final Fill and Year-End Adherence Bonuses (Coming Later This Year): Additional incentives for successful adherence management throughout the course of measurement year.

How to Access LAMP Opportunities:

  • Look for the bull’s-eye icon on the left side of your EQUIPP dashboard. This indicates available opportunities.
  • Click the bull’s-eye and select the Longitudinal Adherence Monitoring program.
  • Eligible patients and their available interventions will appear under the Document Status column.
  • Click expand next to a patient to view all available interventions.
  • Document the intervention in EQUIPP to receive payment.

2. Controlling Blood Pressure Program: Incentives for Documented Readings

Participate in our program and receive incentive payments for electronically recording blood pressure readings for eligible patients. Empower your credentialed team members to conduct patient blood pressure interventions and screenings and document their findings within the EQUIPP platform.
